- Technical Assistance:
The licensing inspector conducted an unannounced monitoring inspection on November 29, 2018 from approximately 4:10 PM until 6:00 PM. There were a total of 104 children present at the center with seven staff supervising the children. During the inspection, children were observed completing homework, eating an afternoon snack, taking bathroom breaks and washing hands, reading books, playing games on the computer, working puzzles, building with legos and playing pool and air hockey. The afternoon snack consisted of pepper steak and rice, carrots, apples and milk. All areas of the center including the classrooms, common area, hallways and bathrooms were inspected. The outdoor area was not inspected due to inclement weather. The center was equipped with toys and supplies and items were available to the children. The following information was reviewed: emergency supplies, emergency preparedness plan, fire and shelter-in-place drills, daily attendance, menu, required postings and parent agreements. A member of management stated medication is not administered. During the inspection, 10 children records were reviewed. Staff information required to be present at the center for staff members were reviewed. Complete staff records are stored at the Human Resource office and will be reviewed at a later date. Standard #: 22VAC40-185-60-A Description: Based on the review of 10 children's records, one record did not contain the required information. Evidence: During the inspection, the licensing inspector observed the record for Child #5 did not contain the physical address for one of the two listed emergency contacts. Plan of Correction: I will ask the parent for the address for the second emergency contact.
Standard #: 22VAC40-185-270-A Description: Based on observation, the center did not ensure all areas were maintained in a safe and operable condition. Evidence: During the inspection, the licensing inspector observed large patches of peeling paint on the walls of the dance and art room. Plan of Correction: We will contact the maintenance person and have them sand the walls where the paint is peeling.
Standard #: 22VAC40-185-280-B Description: Based on observation, the center did not ensure all hazardous substances were kept in a locked place using a safe locking method that prevented access by children. Evidence: During the inspection, the licensing inspector observed the kitchen door propped open. A variety of cleaning materials were located in an unlocked cabinet, underneath the sink in the kitchen. Plan of Correction: Cleaning materials will be moved to the locked closet.
