Advised center of soon-to-expire medication.
An unannounced monitoring inspection was conducted today from 9:30am-11:50am. There inspector observed 42 children directly supervised by 6 staff. The physical plant, 5 staff records, 5 children?s records, 5 medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, transitioning to snack and to martial arts class. Positive interactions were observed throughout the inspection. There was an abundant supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-240-D-4 Description: Based on record review and staff statement, one child for whom emergency medications have been prescribed was not be in the care of a staff member who was trained and certified to administer the medication. Evidence: Child 9 has emergency medication present at the center and currently only Staff 6 is trained to administer the medication. Staff 6 was not present on the date of inspection and will return on 12/3/2018. Plan of Correction: We will get additional staff members trained to administer the medication.
Standard #: 22VAC40-185-280-B Description: Based on observation, not all hazardous substances were kept in a locked place using a safe locking method that prevents access by children. Evidence: A bottle of spray cleaner and a canister of aerosol disinfectant were observed in an unlocked closet in Room 110. Plan of Correction: We will have the teacher engage the lock on the closet.
Standard #: 22VAC40-185-510-A Description: Based on a review of medication, one non prescription medication held by the center did not have the appropriate authorization. Evidence: The medication type present for Child 10 did not match the medication type of the medication authorized by the doctor. Plan of Correction: The medication and a new authorization form will be sent home with the parents for revision.
Standard #: 22VAC40-191-60-C-2 Description: Based on record review, two staff reviewed did not have the results of a central registry check within 30 days of employment. Evidence: Staff 1 and Staff 5's records did not contain a central registry finding. Plan of Correction: The requests have been submitted and we will follow up with the agency or submit new background checks as needed.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on record review, the child day center did not obtain a copy of the results for staff of a search of the central registry or equivalent registry maintained by any other state in which the individual has resided in the preceding five years for any founded complaint of child abuse or neglect against him or her. Evidence: Staff 1's record did not contain a central registry check from a previous state of residence. Plan of Correction: The required form was printed during the inspection and completed. It will be mailed out as soon as possible and the results filed in the staff record when they are returned to the center.
