Technical Assistance:
Consultation provided on being consistent in documenting the time of injuries and the time parents were notified of injuries to their child.

Comments:
An unannounced monitoring inspection was conducted today with the director, who was present with one additional staff and 12 children at the end of the inspection. The children were engaged in the enrichment areas during the inspection. Thirteen medications were reviewed for seven children. Four staff and seven children's files were reviewed as well. Arrival time: 6 am Departure time: 7: 30 am

Violations:
Standard #: 22VAC40-185-160-C
Description: Based on a review of four staff files and an interview with the director, it was determined that two staff did not have up to date tuberculosis test/screenings every two years as required. Evidence: Staff A's tuberculosis test is dated 11/11/16 and staff B's tuberculosis test is dated 9/28/16.

Plan of Correction: We will have the staff update their tuberculosis tests/screenings.

Standard #: 22VAC40-185-510-G
Description: Based on a review of 13 medications for 7 children and an interview with the director, it was determined that one child's prescription medication was not maintained with the original pharmacy label. Evidence: Child A's Ventolin Inhaler was not in the original pharmacy box/label.

Plan of Correction: We will obtain the pharmacy box from the parent.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on a review of staff files and an interview with the director, it was determined that one staff did not have the out of state child abuse and neglect search request nor was there any documentation that the request was sent off within 30 days or the status checked on 45 days thereafter. Evidence: Staff A lived in two states other than Virginia in the last five years and there was no information that those child abuse and neglect searches have been completed.

Plan of Correction: I will check with Corporate to see if we have that information and if not we will send off for those background checks within 10 days.
