The licensing inspector conducted an unannounced monitoring inspection Thursday 29, 2018 from 11:10am to 1:20pm. The children were observed eating lunch. Lunch today consisted of chicken nuggets, mixed vegetables, potatoes, topical fruit, bread and milk. The menu was posted. The infants were observed playing with toys, being fed and being held. All classrooms and playgrounds were inspected today as well as the facility?s mini buses. Five children?s records and five employee records were reviewed during this inspection. The center administers medication; however they did not have any being given at this time. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 10/16/2018 Last shelter-in-place drills: 09/17/2018 Last fire inspection: 12/11/2017 Last health inspection: 01/23/2018 Total enrollment today: 55 Today, the following child to staff ratios were observed: Infants ?20:5 Toddlers ?14:3 Twos? 16:2 Threes ?10:1 Pre-K-15:2 School Age ? not present The violations from the previous inspections were checked for corrections. There were no repeat violations found during today?s inspection. If you have any questions about this inspection, please contact the licensing inspector, Roshon Casey, at (804) 662-9760. PROVIDER: Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Standard #: 22VAC40-185-140-A Description: Based on a review of children's records on 11/29/2018, the center did not ensure that one of four records reviewed contained a physical before or within one month after attendance. Standard #: 22VAC40-185-60-A Description: Based on a review of children's records on 11/29/2018, the center did not ensure that when applicable, two of four records reviewed contained documentation of the work phone number of each parent. Standard #: 22VAC40-185-520-A Description: Based on observation on 11/29/2018, the center did not ensure that over-the-counter skin products shall not be kept beyond the expiration date of the product. EVIDENCE: 1. A tube of CVS Healing Ointment diaper rash cream belonging to Child #4 was observed in the twos room. The cream expired 03/2018. 2. A tube of Bourdeaux Butt Paste diaper rash cream belonging to Child #5 was observed in the infant room. The cream expired 09/2017. Plan of Correction: Not available online.
