Comments:
Thank you for your time and assistance with the unannounced monitoring inspection conducted on November 28, 2018. The on-site inspection was from 12:00p.m. to 4:50p.m. On this date, at the time of arrival, there were 32 children present in three classrooms and in the care of six staff members, with one extra staff member available to assist, if needed. There were 49 children enrolled in the program. Five staff members' records and five children?s records were reviewed. All three classrooms were observed. The medication and medication paperwork were reviewed for three children. There were four staff members and two children interviewed. The exceptions to compliance were noted on the violation notice and the risk ratings were noted on the supplemental page. All interactions between staff and children were age-appropriate and positive. The current themes for the different age groups were The Holidays, The End of Fall and Beginning of Winter, and Decision Making. Lunch was observed to meet the USDA guidelines and consisted of bagel pizza, garden salad, pineapple, and milk. Some items observed and reviewed during the inspection included: required posted information, updated emergency preparedness plan, emergency evacuation and shelter-in-place drills, diaper ointments and parent authorizations, injury reports, first-aid kit and emergency supplies, annual health inspection, outdoor play, the playground, story time, choice time, diaper changing procedures, hand washing and bathroom procedures, lunch, and nap time. Violations:
Standard #: 22VAC40-185-260-A
Description: Based on review of records and verification from staff, the center failed to obtain a fire inspection within 12 months of the most recent one. Evidence: 1. The date of the most recent fire inspection was 10/10/2017. 2. Staff reported the fire marshal had not been to the facility within the last year.

Plan of Correction: During the inspection, the director called and left a message with the fire marshal. She will follow-up and request an inspection to take place as soon as possible. Once received, she will send the new report to the licensing inspector.

Standard #: 22VAC40-191-60-C-2
Description: Based on review of five staff members' records and confirmation from the director, the center failed to obtain the results of a search of the central registry for two new staff members within 30 days of employment. Evidence: 1. Date of hire for Staff Member #1 was 06/04/2018 and the results of the search of the central registry check were received on 09/21/2018. The request to obtain the search was mailed on 08/06/2018. 2. Date of hire for Staff Member #2 was 06/20/2018 and the results of the search of the central registry check were received on 11/27/2018. The request to obtain the search was mailed on 10/15/2018.

Plan of Correction: The director now understands that the results of a central registry check must be received within the first 30 days of employment. She will ensure that the requests to have the searches completed for all new staff will be mailed within a week of employment.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on review of five staff members' records and confirmation from the director, the center failed to obtain the results of the national fingerprinting criminal history record check for three new staff members before they began employment. Evidence: 1. Date of hire for Staff Member #1 was 06/04/2018 and the results of the fingerprint check were received on 07/03/2018. 2. Date of hire for Staff Member #2 was 06/20/2018 and the results of the fingerprint check were received on 08/31/2018. 3. Date of hire for Staff Member #3 was 06/28/2018 and the results of the fingerprint check were received on 07/03/2018.

Plan of Correction: The director now understands that a staff member can not be hired before the results of the national criminal history record check are received. She will ensure that no staff member begins employment before the results are received.
