An unannounced renewal inspection was conducted today from 11:00am-2:30pm. There were 48 children (infants-4yrs.) directly supervised by 11 staff. The physical plant, 5 staff records, 6 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, listening to stories, attending cooking class, making orange juice and fruit salad, coloring a flag, learning about maps, states, and countries, playing with play-doh, building with blocks and eating lunch. Today's lunch consisted of eggs, peas, carrots, brown rice, pears and milk. Hand-washing procedures were also observed. There was an abundant supply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified on the Violation Notice. Standard #: 22VAC40-185-130-A Description: Based on record review, the center did not obtain immunization documentation for each child prior to their first day of attendance. Evidence: The records of Child#5 and #6 did not contain documentation of immunization records. Plan of Correction: The children are no longer enrolled.
Standard #: 22VAC40-185-140-A Description: Based on record review, the center did not obtain documentation of a physical examination for each child within 30 days of first attendance. Evidence: The records of Child#5 and #6 did not contain a physical examination. Plan of Correction: The childenr are no longer enrolled.
Standard #: 22VAC40-185-60-A Description: Based on record review, children's records were incomplete. Evidence: Child#2's record is missing the street name and number of one emergency contact. Child#3's record is missing the father's workplace and the mother's work telephone number. Child#5 is missing the parent's employer information, and the address for both emergency contacts. Plan of Correction: We will get the missing information.
Standard #: 22VAC40-185-70-A Description: Based on record review, the center did not maintain complete staff records. Evidence: Staff#5's record did not contain documentation that two or more references as to character and reputation as well as competency were checked before employment. Plan of Correction: We will get the missing references tonight.
Standard #: 22VAC40-185-260-A Description: Based on record review, the annual fire inspection is expired. Evidence: The last fire inspection was dated 10/18/2018, the posted fire permit expired on 2/23/2018. Plan of Correction: We have scheduled a fire inspection for January 10, 2019.
Standard #: 22VAC40-185-510-C Description: Based on review of medication and provider interview, it was determined that a child had two medications on site without a medication authorization form. Evidence: Child#7 has two long term medications on site. Both of these medications were on site without a medication authorization form completed and signed by the parent and the physician. Plan of Correction: We will give the parent the authorization form today.
Standard #: 22VAC40-185-510-N Description: Based on review of medication and interview, medication with an expired authorization form was not returned to the parent within 14 days of expiration. Evidence: The medication authorization form for a prescription emergency medication for child#8 expired on 11/12/2018. The medication was observed on-site during the inspection. Plan of Correction: We will give the parent a new authorization form.
