Comments:
The licensing inspector conducted an unannounced monitoring inspection on Monday, November 27, 2018 from 9:50am to 1:34pm to determine the center?s compliance with licensing standards. The center?s director was present and assisted the inspector throughout this inspection. The census for today?s inspection consisted of 83 children in care and approximately 14 staff. During this inspection, the licensing inspector observed the children during circle time, playing in centers, and out in the playground. The children were also observed interacting with peers and with staff. The staff were observed having positive interactions with the children. Lunch today consisted of Hungarian pork stew with peas and carrots, corn bread, diced pineapple, and milk. During the inspection, the required postings were reviewed and found to be in compliance. The last health inspection occurred on 09/25/2018. The last fire inspection occurred on 05/02/2018. Eight staff records and eight children?s records were reviewed during this inspection. Violations from the previous inspection were reviewed for correction. Last evacuation drill: 11/19/2018 Last shelter-in-place drill: 07/05/2018 and 09/16/2018. Standard #: 22VAC40-185-130-A
Description: Based on a review of eight children's records on 11/27/2018, the center did not obtain documentation that one of eight children received the immunizations required by the State Board of Health before the child can attend the center. EVIDENCE: 1) The record for Child #1 (DOE: 01/02/2018) contained documentation of immunization dated 01/10/2018.

Plan of Correction: Per the Center: Going forward management will require that we have this in hand before the child is able to start.

Standard #: 22VAC40-185-140-A
Description: Based on a review of eight children's records on 11/27/2018, the center did not ensure two of eight children's records reviewed contained documentation of a physical examination before the child's attendance; or within one month after attendance. EVIDENCE: 1) The record for Child #2 (DOE: 09/03/2018) did not contain a physical examination. 2) The record for Child #7 (DOE: 07/30/2018) did not contain a physical examination. Each child shall have a physical examination by or under the direction of a physician before the child's attendance; or within one month after attendance.

Plan of Correction: Per the Center: Going forward management will require that we have this in hand before the child is able to start.
