An unannounced monitoring visit and facility tour was conducted from 12:15pm- 1:25pm. At the time of inspectors' arrival there were 39 pre-school and school aged children in care with 6 staff members and 1 administrator. Children were observed participating in indoor play, nap, transitioning between activities, and hand washing. A sample of 5 children's records and 7 staff on-site records were reviewed. Injury reports and medications were also reviewed. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.
Standard #: 22VAC40-185-140-A Description: Based on record review and interview it was determined the center did not ensure each child had a physical exam by or under the direction of a physician within the required time frames. Evidence: 1. The record for child #3(enrolled 9/4/18) did not contain documentation of a physical exam. 2. Staff confirmed that this documentation was not present during this inspection. Plan of Correction: Staff will request this documentation from parent at pick up.
