Comments:
An unannounced monitoring inspection was conducted on December 3 , 2018 from 9:40am until 12:30pm. Upon arrival to the facility, there were 69 children present with 14 staff members. The children were observed participating in age appropriate activities, hand washing and diapering. A sample of 6 children's records and 6 staff records were reviewed. The licensing inspector toured the facility and reviewed supervision, emergency supplies, practice drills, nutrition, physical plant, required postings, and administration. There was a violation cited in the area of physical plant. An exit meeting was conducted with the facility director prior to the end of the inspection.

Standard #: 22VAC40-185-280-B
Description: Based upon observation the provider did not ensure that hazardous substances were kept in a locked place using a safe locking method that prevents access by children. If a key is used, the key shall not be accessible to the children. Evidence: 1. During the inspectors tour of the facility it was found there were unlocked cabinets containing hazardous substances. 1. The unlocked cabinets containing hazardous substances were located under the sink in the two year old room. a. The unlocked cabinets containing hazardous substances could be a potential hazard to children. b. There were children in the two year old room during the time the violation was observed. c. The hazardous substances were labeled keep out of reach to children. d. The director of the facility toured the two year old room with the inspector and also identified and confirmed the violation.

Plan of Correction: Staff will removed hazardous substances from cabinet.
