Technical Assistance:
Discussed with center management: Annual updates for children's records, Follow up on background checks, information on recalled equipment and MAT forms.

Comments:
An unannounced monitoring inspection was conducted from 9:35 am-11:45am. 128 children were in care with 21 staff members present. Children were observed engaged in several group activities such as writing on worksheets, doing circle time, playing with toys, singing songs and listening to books on tapes. 5 staff and 5 children records, 2 medications and first aid kits on each level of the building were reviewed. Violations:
Standard #: 22VAC40-185-190-A
Description: Based on record review and staff interview, the program director did not meet the qualifications required for a center director. Evidence: 1. Staff # 1 was hired as the center director on 11/27/18. Staff # 1's record did not have any documentation to indicate that she met the requirements to be a qualified center director. 2. Staff # 1 stated that she has her high school diploma but a copy was not available for review. In addition to an absence of written documentation, Staff # 1 also did not have any supervisory experience.

Plan of Correction: I will get a copy of the high school diploma and the staff person will be registered for a CDA credential.

Standard #: 22VAC40-185-350-E-2
Description: Based on observation, the ratio of 5 children to 1 staff was not maintained in the toddler classroom. Evidence: 1. It was observed that Staff # 2 stepped out of the toddler room to return the vacuum. 2. When Staff # 2 left the classroom, there were only 2 staff members left with 19 children. The children were between the age 16 months to 24 months. 3. The two staff members were left over ratio for approximately 1 minute.

Plan of Correction: I will remind the staff to maintain the required ratio.

Standard #: 22VAC40-185-430-N
Description: Based on observation, recalled equipment was still being used in the center. Evidence: 1. 4 bumbo seats were seen in the infant classroom. An infant was observed using one of the four bumbo seats. 2. None of the four bumbo seats had a safety restraint attached to the seat as is now required.

Plan of Correction: We will pull the seats out of the classroom and look for replacements.
