Comments:
An unannounced renewal inspection was conducted today from 10:30am-1:15pm. There were 58 children (infants-4yrs.) directly supervised by 12 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, listening to a story, playing outside, being bottle fed, napping, coloring, singing, and eating lunch. Diapering and hand-washing procedures were also observed. There was an abundant supply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified on the Violation Notice. Standard #: 22VAC40-185-270-A
Description: Based on record review, areas and equipment of the center were not maintained in a safe and operable condition. Evidence: In the two's classroom, the cabinet under the sink used by the children has areas of paint that are peeling off on the right side of the cabinet and on the doors, and the cabinet doors are coming loose.

Plan of Correction: We will replace that cabinet.

Standard #: 22VAC40-185-510-N
Description: Based on review of medication and interview, medication with an expired authorization form was not returned to the parent within 14 days of expiration. Evidence: The medication authorization form for a prescription allergy medication for child#1 expired on 6/28/2018. The medication was observed on-site during the inspection, the medication also expired on 08/2018.

Plan of Correction: We will send the medication back to the parents today.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on staff record review, there is no documentation that Child Abuse and Neglect checks required within 30 days of employment was initiated for an employee that has resided outside the state of Virginia within the last 5 years. Evidence: Staff#1 (DOH: 05/07/2018) has resided outside the state of Virginia within the last 5 years, their records did not have documentation that out-of-state child abuse and neglect results were obtained.

Plan of Correction: We will check on the status of the out-of-state background check.
