A complaint investigation was conducted today regarding physical abuse of a 3-year old and a 5-year old child. Interviews were conducted, and documentation was reviewed. There was not a preponderance of evidence to support the allegation regarding physical abuse and the complaint was deemed invalid. However, two non-related violations were found and are identified in the Violation Notice. Standard #: 22VAC40-185-60-A Complaint related: No Description: Based on record review, 1 of 2 children's records did not contain all of the required information. Evidence: Child #2's (start date: 9/4/2018) record did not contain documentation of a complete address for either of the two required emergency contact persons. Plan of Correction: For currently enrolled children, we will make sure to request that information before they start.
Standard #: 22VAC40-185-280-B Complaint related: No Description: Based on observation, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children. Evidence: In the Rockets classroom (children ages 3-5 years), a container of lysol wipes and hand sanitzer was sitting on top of the teacher cabinet. Plan of Correction: It was corrected today, and we will send a memo to the staff to remind them that all chemicals need to be locked.
