- Technical Assistance:
The licensing inspector conducted an unannounced monitoring inspection Tuesday, December 4, 2018 from approximately 10:00 AM until 3:20 PM. The following ratios were observed with a total of 50 children present during the inspection: Early Head Start (1-2 years) - 6 children with 2 staff Early Head Start (2 years) - 7 children with 1 staff Head Start (Threes & Fours) - 12 children with 2 staff Preschool I (Infants ? 2 years) - 11 children with 4 staff Preschool II (Threes & Fours) - 14 children with 2 staff The children and staff were engaged in a variety of activities when the licensing inspector arrived. Children were observed being held and fed, looking at books, playing in the gym, playing outside, creating Christmas trees, singing songs, taking bathroom breaks, washing their hands and eating lunch and resting at nap time. Lunch consisted of beans and rice burrito with cheese, corn, blueberries and strawberries and milk. All areas of the center including classrooms, hallways, kitchen, bathrooms, gym and playgrounds were inspected. Measurements of resilient surfacing material (mulch) were not taken due to moisture levels. The center was equipped with toys and supplies and items were available to the children. The following information was reviewed: injury reports, daily sheets, Infant feeding schedules, practiced emergency drill logs, emergency supplies, parent agreements, staff training information and liability insurance. The center?s first aid kit and emergency supplies were inspected. Medication is administered and medication and authorization forms were reviewed. Last documented health inspection: 11/05/18 Last documented fire inspection: 12/19/17 During the inspection, five children records and five staff records were reviewed. Standard #: 22VAC40-185-160-A Description: Based on the review of five staff records, two staff members did not obtain documentation of a negative tuberculosis screening within the required time frame. Evidence: During the inspection, the licensing inspector observed the record for Staff #1 (hire date 11/14/18) contained a tuberculosis screening date 11/14/18. The record fro Staff #3 (hire date 09/18/18) did not contain documentation of a tuberculosis screening. Plan of Correction: From here going forward, we will ensure all staff have the required tuberculosis screening prior to staff working.
Standard #: 22VAC40-185-60-A Description: Based on review of five children's records, three records did not contain the required information. Evidence: During the inspection, the licensing inspector observed the records for Child #1, Child #4 and Child #5 did not contain the required name, addresses and phone numbers for at least two emergency contacts other than the parents. Plan of Correction: We will ensure information is obtained at the time the children are enrolled.
Standard #: 22VAC40-185-290-3 Description: Based on observation, the center did not ensure all electrical outlets contained protective covers. Evidence: During the inspection, the licensing inspector observed the outlets in the gym did not contain protective covers. During the inspection, there were 14 preschool children playing in the gym. Plan of Correction: We will discuss the requirements of outlet covers with all parties who use the gym to ensure the outlet remain covered.
Standard #: 22VAC40-185-510-C Description: Based on review of medication authorization and medications, the center did not ensure long term medication authorizations contained the required information. Evidence: During the inspection, the licensing inspector observed the medication authorization for a prescribed long-term medication did not contain the parent's signature. Plan of Correction: We will request the parent complete the parent's portion of the medication authorization form.
