An unannounced monitoring inspection was conducted with the director and assistant director beginning at 1:00 PM and ending at 3:25 PM. There were eight 3 year old children in the care of two staff members. The children were observed playing with play dough, dolls, building blocks, and cars. Ten children's records were reviewed. Four staff records were reviewed. There were no medications on site to be reviewed. The outside recreational area was not inspected during this inspection due to weather conditions. The emergency preparedness plan was reviewed and in compliance. The most recent emergency escape drill was documented as 11/19/18. The most recent health inspection was documented as 4/13/18. Standard #: 22VAC40-185-140-A Description: Based on a review of ten children's records, the center did not have documentation of physical in each child's file prior to or within 30 days of first attendance. Evidence: Child A, Child B, Child C (start date 9/5/2018) records did not have documentation of a physical. Plan of Correction: The center will obtain the missing documentation from the parents and place it in the children's records.
Standard #: 22VAC40-185-290-3 Description: Based on observation, not all of the electrical outlets had protective covers. Evidence: There were two electrical outlets in the preschool classroom that did not have protective covers. Plan of Correction: The center will replace the missing outlet covers.
