An unannounced renewal study was conducted today from 10:30am-3:00pm. A total of 133 children were present with 26 staff. Infant feeding, play with sensory bottles, diapering, and handwashing were observed. Toddlers were observed during gross motor play activities in the movement room. Two's were observed during outdoor play. Preschool aged children were observed engaged in various activities, including ornament decorating, storytime, and while positively engaged in interactions with staff. A sample of 12 children's records, 9 staff records, emergency drills, emergency supplies, medications and injury reports were reviewed. Areas of non-compliance are documented on the violation notice. Standard #: 22VAC40-185-130-B Description: Based on a sample review of records, 2 out of 6 records for children under the age of two years old, did not contain documentation of additional immunizations every six months. Evidence: 1. The most recent record of immunizations obtained for Child #11 [14 months old] was obtained 1/17/2017. 2. The most recent record of immunizations obtained for Child #12 [11 months old] was obtained 4/17/2018. Plan of Correction: We will request the information from the parents.
Standard #: 22VAC40-185-270-A Description: Based on observations, an area inside the center was not maintained in clean and safe condition. Evidence: An insect trap was observed in an unlocked/unsecured lower cabinet in an infant room. Plan of Correction: We will have a latch installed on the door immediately.
