An unannounced monitoring visit and facility tour was conducted from 2:05pm-3:45pm. At the time of inspectors' arrival there were 24 preschool aged children in care with 5 staff members and 1 administrator. Children were observed participating in nap and after care program activities. A sample of 5 children's records and 5 staff records were reviewed. Medications and injury reports were also reviewed. Areas of noncompliance are identified in the Violation Notice and were discussed with the Director at the conclusion of this inspection.

Standard #: 22VAC40-185-130-A Description: Based on record review and interview it was determined the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Evidence: 1. The record for child #1(enrolled 8/21/18) did not contain documentation of immunizations. 2. Child #1 was in care during this inspection. 3. The staff confirmed this documentation was not present during this inspection. Plan of Correction: Staff will request immunization documentation from the parent.
Standard #: 22VAC40-185-140-A Description: Based on record review and interview it was determined the center did not ensure that each child had a physical examination by or under the direction of a physician within the required time frames. Evidence: Evidence: 1. The record for child #1(enrolled 8/21/18) did not contain documentation of a physical exam. 2. Child #1 was in care during this inspection. 3. The staff confirmed this documentation was not present during this inspection. Plan of Correction: Staff will request documentation of a physical exam from the parent.
Standard #: 22VAC40-185-160-C Description: Based on record review it was determined all staff did not resubmit TB test results at least every two years from the date of the first initial screening or testing. Evidence: 1. The record for staff #1(hired 8/24/15) did not contain documentation of updated TB test results. The documentation available was dated 9/3/16. 2. The record for staff #2(hired 11/15/10) did not contain documentation of updated TB test results. The documentation available was dated 5/18/16. 3. The record for staff #3(hired 8/1994) did not contain documentation of updated TB test results. The documentation available was dated 6/2/16. Plan of Correction: Staff will get updated TB tests or screenings.
