Comments:
A renewal study conducted with the owner of the facility and the director, There were 80 children in care, the age range of the children was three months to nine years old. Children were observed during free play, outside time. Five staff are current in MAT and three staff were current in Daily health observation. Standard #: 22VAC40-185-60-A
Description: Based on the review of ten staff files not all were complete. Evidence: Child A with a start date of 8/6/18 was missing the work number for father. Child B with a start date of 9/20/2018 was missing work number for father. Child C with a start date of 5/5/18 was missing the street address and work number for fathers employment.

Plan of Correction: We wil ask the parents for the missing information.

Standard #: 22VAC40-185-270-A
Description: Based on observation not all areas on the playground were in a safe and operable condition. Evidence: There was green netting sticking up thru the grass on the Toddler play ground posing a tripping hazard.

Plan of Correction: Inspect the entire playground after the snow melts and cut the green needing. Until then playground is off limits.

Standard #: 22VAC40-185-350-E-1
Description: Based on LI's observation and interviews with class room staff the center failed to stay with in a one staff member to four infants ratio in the infant room. Evidence: In the infant room there were nine infants with two staff. A third staff came and and stated they were out of ratio for ten minutes.

Plan of Correction: Always monitor and keep in ratio. Corrected that day.

Standard #: 22VAC40-185-370-5
Description: Based on interviews with the infant staff, the center was not documenting tummy time for infants that cannot turn themselves over. Evidence: Tummy time was not documented for a four week old infant.

Plan of Correction: Have document placed in class folder to record daily tummy time.
