Comments:
10/16/2018 Arrived 2:15pm Departed 6:05pm An unannounced renewal inspection was conducted today. Fifty-six children were present with six staff. The children were playing games, doing homework, playing in the gym and working in the computer lab. Previous violations, medication, staff and children's files were reviewed. Violations:
Standard #: 22VAC40-185-510-A
Description: Based on the review of medications on site, the facility failed to insure that there is written permission from the parent for an emergency medication. Evidence: 1. There is an Epi pen on site for child #1. There is no written authorization from the parent.

Plan of Correction: Parent will be given back the Epi pen and informed they must provided an authorization form.

Standard #: 22VAC40-185-510-E
Description: Based on a review of medications, the facility failed to insure that all medicine is labeled with the child's name. Evidence: 1. There is an Epi pen in the medication box and it is not labeled with a name.

Plan of Correction: Epi pen will be disposed of at a local pharmacy.

Standard #: 22VAC40-185-510-N
Description: Based on the review of medications on site, the facility failed to insure that medications are returned to the parents when no longer needed. Evidence: 1. There are two prescription medicines in the medication safe which were used by children in the summer program and have not been returned to the parents or discarded.

Plan of Correction: Medications will be returned to parents.
