11/27/2018 Arrived 10:00am Departed 1:15pm An unannounced monitoring inspection was completed today. Ninety-two children were in care with 17 staff. The age range of children in care is 6 weeks to five years. Children were having group time, playing in centers, eating lunch and resting. Previous violations, medications, eight children's files and eight staff files were reviewed. You reported that the playground is not being used until the renovations are complete. Description: Based on a tour of the infant room the facility failed to maintain a temperature of no less than 68 degrees. Evidence: 1. The temperature in the infant room was 64 degrees on the floor near the outside door and in the middle of the room. 2. It was 66 degrees on the tray of a baby swing.

Plan of Correction: The director will insure that weather stripping is installed on the exit door and will monitor the temperature in the infant room.

Description: Based on a tour of the building, the facility failed to insure that the hazardous substances are all kept in a locked area. Evidence: 1. In the twos' room there is a can of aerosol Great Value disinfectant on the sink. 2. In the threes' room there is a container of Lysol wipe and a bottle of aerosol Great Value disinfectant on the sink. In the unlocked cabinet above the sink, there are four bottles of liquid. They are labeled bleach water and Lysol water and dated 10/15/18. 3. In the STEM room the unlocked cabinet under the sink contains: aerosol Great Value disinfectant, Windex, hand sanitizer, Lysol and water mixture, two aerosol Great Value air fresheners. 4. In the Pre-K room there are Clorox wipes on the sink. In the unlocked staff bathroom there are Members' Mark disinfecting wipes on a shelf and aerosol Great Value disinfectant. 5. In the unlocked staff bathroom in the cafe, there is a container Good and Clean disinfecting wipes. 6. In the kitchen under the sink there is: a bottle of Pine Sol, a blue liquid which is not labeled and a can of Raid Ant and Roach spray. The kitchen door is open and the cabinet is not locked.

Plan of Correction: The director has installed a magnet lock in the STEM room. A magnet lock for the kitchen has been ordered and will be installed. The director has spoken to all staff to make sure all hazardous susbstances are kept in a locked area.

Description: Based on a tour of the building, the facility failed to insure that all cleaning materials and insecticide/pesticides are stored in areas physically separate from each other. Evidence: 1. In the kitchen cabinet under the sink Raid, an insecticide, is being stored with cleaning products.

Plan of Correction: The director removed the can of Raid from the kitchen and placed it in a locked closet in the hallway.

Description: Based on the tour of the building, the facility failed to insure that the diapering regulations are being followed. Evidence: 1. In the threes' room there are pieces of paper, chalk, crayons and small toys on the diapering table. 2. Diapers are being changed in the bathroom, not on the changing table. 3. The trash can for diaper disposal is hand operated.

Plan of Correction: The director talked with staff and they removed all items from the top of the diapering table. The diapering table will be used for children under three. The director purchased a trash can with a lid that is operated by a foot pedal operated.
