Comments:
The licensing inspector conducted an unannounced monitoring inspection, at which play groups, snack time? were observed. Children were observed; and staff and children files and other documentation were reviewed. 25 children were found to be in care. Previous violations were reviewed, and two (2) of the violations from the previous inspection had not been corrected. (Add areas that continue to need improvement) Also, (add that license expires in ?x? months and it is critical for you to demonstrate improved performance as soon as possible.) Violations:
Standard #: 22VAC40-185-160-C
Description: Based on review of staff files the facility failed to maintain compliance with this standard as required relating to renewal of TB tests or screenings. Evidence: Staff record #2 had an expired TB test, with no renewed test to date. The test on file expired as of 10/18/16. TB tests/screenings are to be renewed every two years.

Plan of Correction: A copy of the most recent test will be obtained and placed within the file. Renewals will occur every two years as required.

Standard #: 22VAC40-185-60-A
Description: Based on review of children files the facility failed to maintain compliance with this standard as required relating to parental information, and allergies. Evidence: Child record #1, enrollment date 11/12/18, did not address the allergies section, or the fathers home address on the enrollment form as required.

Plan of Correction: This information will immediately be obtained and recorded in the file. Files will continue to be verified for compliance.
