A Subsidy Health and Safety inspection was conducted on 12/18/18 to review supplemental health and safety requirements. Licensing Inspector arrived at 10:00 and departed at 10:45 AM. At the time of the inspection, there were two children (infant through two years) in care with two staff. Children were observed participating in free play activities. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: 22VAC40-665-540-A Description: Based on review of three staff records it was determined that the vendor did not ensure that all staff have a statement stating that they are free of communicable tuberculosis (Tb) prior to coming into contact with children. Evidence: Staff #3 did not have documentation that they were free from Tb on file. Staff #1 stated that staff #3 has been working with children. Plan of Correction: Center director will ensure that all staff have Tb screenings on file prior to working with children.
