An unannounced monitoring inspection was conducted today from 11:00am-2:15pm. There were 56children (3-5yrs.) directly supervised by 8 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, playing outside, weather related activities and eating lunch. Hand-washing procedures were also observed. An inspection was completed for the playground, equipment was in good working condition. There was an abundant supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified on the Violation Notice. Standard #: 22VAC40-185-70-A Description: Based on records review, staff records were incomplete. Evidence: Staff #5's record is missing two references as to their character and reputation, as well as competency required prior to the first day of employment. Plan of Correction: We will check records and record them in staff file.
Standard #: 22VAC40-185-290-3 Description: Based on observation, not all unused electrical outlets had child-resistant protective covers. Evidence: In the Green Room, three unused electrical outlets were observed without protective covers. Plan of Correction: Corrected during inspection.
Standard #: 22VAC40-185-550-D Description: Based on review of emergency drills and interview, documentation of evacuation drills was found to be missing or incomplete. Evidence: Documentation of evacuation drills was missing for the months of March and May 2018. Plan of Correction: We completed drills but failed to record them. We will more accurately record drills.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on record review, the center did not obtain a fingerprint result prior to the staff member's first day of employment. Evidence: Staff #2's record (DOH:09/10/2018) contained a fingerprint result dated 11/27/2018. Fingerprint results were not obtained prior to the staff's first day of employment. Plan of Correction: Make sure that finger print results are received before staff's first day.
