An unannounced monitoring inspection was conducted today from 1:00pm-3:15pm. There were 23 children (infants-4yrs.) directly supervised by 5 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed napping, and eating snack. There was an abundant supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified on the Violation Notice. Standard #: 22VAC40-185-60-A Description: Based on record review, the center did not maintain complete children's records. Evidence: Child#3's record is missing he address of one of two required emergency contacts. Child#5's record is missing the mother's work telephone number. Plan of Correction: We will contact the families and request the numbers today.
Standard #: 22VAC40-185-510-N Description: Based on review of medication and interview, medication with an expired authorization form was not returned to the parent within 14 days of expiration. Evidence: The medication authorization form for a prescription allergy medication for child#1 expired on 11/10/2018. The medication was observed on-site during the inspection. Plan of Correction: We sent an email on Friday to the parents to submit the new form. They will bring it back by Friday.
