Comments:
A Subsidy Health and Safety inspection was conducted on (12/17/2018) to review supplemental health and safety requirements. An unannounced monitoring inspection was conducted today from 1:00pm-3:15pm. There were 23 children (infants-4yrs.) directly supervised by 5 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed napping, and eating snack. There was an abundant supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified on the Violation Notice. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: 22VAC40-665-520-B-11
Description: Based on record review, the center did not obtain a written allergy care plan for a child with a diagnosed food allergy. Evidence: Child#1has an allergy medication on-site, the child's record does not contain an allergy care plan to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.

Plan of Correction: We have already requested one via email, we will be getting one on Friday.

Standard #: 22VAC40-665-530-4
Description: Based on record review, the center did not ensure each staff member had a first aid and cardiopulmonary resuscitation (CPR) training as required by the responsibilities held by the staff member. Evidence: Staff#5's record did not contain a CPR and First Aid training certification.

Plan of Correction: We will schedule a class.

Description: Based on review of the emergency plan, the center's emergency response plan did not include procedures for the continuity of operations to ensure that essential functions are maintained during an emergency.
