A self reported incident that occurred on 10/24/2018 was investigated on 11/01/18 regarding sight and sound supervision, care and protection and policies and procedures. Standard #: 22VAC40-185-40-E Description: Based on review of the center's policies and procedures and staff interviews, it was determined that the center did not ensure that staff followed the center's own policies and procedures required by the standards for supervision, and playground safety rules. Evidence: The center's policy on supervision and playground safety rules states: Children must always be within sight and sound of staff while attending CASA, and that staff must situate themselves so children are in sight at all times. On the incident day of 10/24/2018, before leaving the playground to go back into the building Staff#1 conducted a head count of children. At some point after the children lined up and they were walking back into the building, Child#1 left the line and went back to the playground. Staff#1. #2 and #3 were unaware that Child#1 was not in their care until Adult#1 walked the child back to the program. Plan of Correction: Discovery Staff are retrained on CASA Playground Attendance Procedure.
Standard #: 22VAC40-185-340-A Description: Based on information obtained during the investigation, it was determined that the center did not ensure than when staff are supervising children, they shall always ensure their care, protection and guidance. Evidence: Staff#1, #2 and #3 confirmed that on the day of the incident, 10/24/2018, they were unaware that child#1, age 6, was not with the rest of the group while the children were washing hands and entering the cafeteria at approximately 4:00pm. Staff#1 stated that they counted children before entering the building, and proceeded to walk the children back into the building to wash their hands before entering the cafeteria. On the day of the incident two parents stopped Adult#1 to assist child#1 after they found the child alone and unattended on the playground. Adult#1 brought Child#1 back to the program, shortly after the children entered the cafeteria. Staff failed to ensure the care and protection of child#1 by not keeping the child within sight and sound supervision at all times. Plan of Correction: CASA Playground Attendance Procedure Roll call after every playground transition. Staff verifies that all students are accounted for before and after each playground transition. *Staff must call attendance before transitioning students to the playground. *Head count is completed after the children exit the building. *Staff must call attendance before transitioning from the playground to inside the building. *Head count is completed after the children enter the building. *During playground transitions there is a staff member in the front and back of the group. Students are escorted by staff to any area that a child is transitioning to. Children are to be in sight and sound at all times.
Standard #: 22VAC40-185-340-F Description: Based on interviews and a review of documentation, it was determined that the center did not ensure that all children 10 years of age or younger shall always be within actual sight and sound supervision of staff. Evidence: On 10/24/2018, at approximately 4:15pm Child#1 was found by two parents alone and out of sight and sound supervision of staff on the school's playground. When the licensing inspector interviewed adult#1 who was alerted by these two parents about the child being outside alone and unsupervised on the date of the incident, adult#1 confirmed that child#1 was outside the program with no staff present. Based on the approximate time when the children lined up to go inside, and the time that they took to wash hands and enter the cafeteria, it was determined that Child#1 was alone and out of actual sight and sound supervision of center staff for approximately 10 to 15 minutes. Plan of Correction: Discovery Staff are retrained on CASA Playground Attendance Procedure.
