- Technical Assistance:
Technical assistance today included physical environment, medication authorizations and the storage of medications, maintenance of resilient surfacing and age appropriate craft items.
An unannounced renewal inspection was conducted today from 11:15 am - 2 pm. There were 57 children present in 6 classrooms. All were in ratio. I observed children playing in centers, listening to a story, singing "The Farmer in the Dell", eating lunch, and preparing for naptime. Record keeping was well-organized. Standard #: 22VAC40-185-270-A Description: Based on observation during the inspection, there were areas and equipment observed to not be maintained in a clean, safe and operable condition as required. Evidence: Indoors- There was chipping and badly peeling paint accessible at child height throughout the center. The schoolage room had a broken moulding on the bottom of the wall; sharp broken edges on the Lego table and missing covers for the rods of the foosball table. In most rooms, trash cans were very dirty. The bottom of the children's sink was deteriorating in one room. The bottom of the diaper table had a missing door, leaving a very dirty shelf exposed at child height. The vent above the diaper table had visible dust. The sprinkler door was unlocked and accessible. A trash can in use today for nondisposable diapers had a sharp triangular-shaped hole the size of a finger accessible at child height. The edge of the outlet/light fixture was accessible in one classroom. In a preschool room, two carpets were observed to be overlapping, causing a tripping hazard. Outdoors: There was a gap observed in the corner of the toddler play area where an area is exposed at ankle level that can become an entrapment. Plan of Correction: Maintenance is here today and I will make him aware of all the concerns. The sprinkler door will always be locked. The broken trash can will be replaced. I will cover the outdoor corner area with temporary barriers so the area is not accessible to children.
Standard #: 22VAC40-185-510-D Description: Based on a review of over the counter medications today, there were some authorizations that did not meet the standards. Evidence: There was no authorization available today for the OTC allergy medication for Child # 1, and the manufacturer's date expired 6/17 on Child # 2's medication. Plan of Correction: We will talk to the parents today.
Standard #: 22VAC40-185-510-G Description: Based on a review of medications today, there were some prescription medications and authorizations that did not meet the standards. Evidence: The allergy medication for Child # 1 did not have a written authorization, although an allergy action plan was present. The allergy authorization for Child # 4 had an expiration date of 10/18. Plan of Correction: I will return these to parents to get current paperwork.
Standard #: 22VAC40-185-510-N Description: Based on a review of medications at the center today, three medications no longer authorized were present at the center. Evidence: The medications for Child # 5, 6 and were present although the authorizations have expired or the children no longer attend. Plan of Correction: We will make sure parents are called and discard the medication after two weeks if parents do not pick them up.
