63.2(17) License & Registration Procedures

An unannounced focused inspection was completed on December 7, 2018 from approximately 10:30a.m. to 12:00p.m. to review compliance with specific standards. Forty-one children were in attendance, ages infant to pre-school age. One classroom space, the infant room, was observed. There has been one new staff hired since the last inspection. Nine staff records were reviewed. The Owner/Director was available for interview and the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgment of Inspection form was left with the licensee. There were three citations for violations of the Standards.

Standard #: 22VAC40-185-240-A Description: Based on a review of staff records, the licensee failed to ensure staff received required training by the end of their first day assuming job responsibilities for one of nine staff. Evidence: 1. Staff #6 was hired on September 27, 2018. 2. Staff #6?s record contained initial training required by the end of Staff #6's first day of assuming job responsibilities dated October 8, 2018. 3. Administrator #1 acknowledged this training was not completed by the end of Staff #6's first day of assuming job responsibilities. Plan of Correction: The Owner/Director will ensure that all new staff receive the required training within the required timeframe.
Standard #: 22VAC40-185-570-A Description: Based on observation, the licensee failed to ensure that when a child is placed in an infant seat or high chair, the protective belt shall be fastened securely. Evidence: 1. The licensing inspector observed a child, approximately one year old, in the infant room, standing up in a high chair. Two staff were observed in the room, one with her back facing the child while writing something down and one wiping the face of another child seated in a high chair next to the child standing up in the high chair. 2. The staff that was wiping another child's face was observed to be watching the child stand up in the chair but not intervening. Plan of Correction: The Owner/Director will be firmer with staff regarding safety standards and will retrain these staff on safety.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on review of staff records, the licensee failed to obtain a fingerprint results prior to employment for one of nine staff. Evidence: 1. Staff #6 was hired on September 17, 2018. 2. Staff #6?s record contained fingerprint results dated October 10, 2018. 2. Administrator #1 acknowledged this staff began employment prior to obtaining the fingerprint results. Plan of Correction: The Owner/Director will not hire new staff without obtaining fingerprint results first.
