An unannounced complaint investigation was initiated in the facility on December 20, 2018, in regards to behavioral guidance and forbidden action. The investigation was conducted with local agency. Interviews were conducted and records were reviewed. Based on a interviews with staff during investigation, a preponderance of evidence does not support the allegation, therefore the complaint is determined NOT VALID.
