Comments:
An unannounced monitoring inspection was conducted on Thursday, December 27, 2018 to determine the center's compliance with licensing standards. The inspector arrived at 10:30 am and concluded the inspection at 3:15 pm. The census for today?s inspection consisted of 78 children. Upon the inspector's arrival, children were engaged in various activities. The K-Prep children were observed cleaning up their centers and preparing to go out to the playground. The K-Prep children were later observed playing outside and interacting with peers and staff. Children in the Toddler classroom were observed taking bathroom breaks and eating lunch. Infants were observed having lunch and interacting with staff. Staff interacted with children in a positive manner. School age children were playing in centers. The playground area was inspected. The classrooms are decorated with the children's artwork and are equipped with age and stage appropriate materials. The center had the following posted: license, daily schedule, emergency numbers, evacuation maps, menu and various parent information. The center's first aid kit and non-emergency supplies were inspected and found complete. Seven children's records and five staff records were reviewed during today's inspection. Emergency medications and authorizations were reviewed. Health Inspection: 07/13/2018 Fire Inspection: 12/07/2018 The last documented evacuation drill was held on 11/30/2018. A shelter-in-place drill was documented on 09/17/2018. A second shelter-in-place drill will need to be completed before the end of the year as two shelter-in-place drills are required annually. The monthly menu was posted and today's lunch consisted of: milk, veggie patty, mixed veggies, apple slices, and whole grain hamburger bun. Standard #: 22VAC40-185-140-A
Description: Based on a review of seven children's records on 12/27/2018, the center did not ensure one child obtained documentation of a physical examination by or under the direction of a physician prior to attendance or within one month of attendance. EVIDENCE: 1) The record for Child #2 (DOE: 02/13/2018) did not contain a physical examination report.

Plan of Correction: Per the Center: I will contact the parent and ask them for a complete address and a current physical.

Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records on 12/27/2018, the center did not ensure that one of seven children's records contained two complete emergency contact records. EVIDENCE: 1) The second emergency contact listed in the record for Child #2 (DOE: 02/13/2018) did not contain a complete address. The address did not contain the state or zip code. Each child record shall contain the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.

Plan of Correction: Per the Center: I will contact the parent and ask them for a complete address.

Standard #: 22VAC40-185-270-A
Description: Based on observation at the center on 12/27/2018, the center did not ensure all areas and equipment of the center were maintained in a clean, safe and operable condition. EVIDENCE: 1) In the Three's classroom, there was a small hole in the lower left corner of the windowsill, surrounded by peeling paint accessible to children. 2) In the bathroom shared by the K-Prep and Three's classrooms, there was a broken soap dispenser that had become detached from the wall. The area where the soap dispenser was originally hung had peeling paint around it which was accessible to the children in care.

Plan of Correction: Per the Center: Will contact [our maintenance man] to come out and fix the part that is chipping in the building. He will come out in the week of Jan 7th.

Standard #: 22VAC40-185-280-B
Description: Based on observation at the center on 12/27/2018, the center did not ensure to keep hazardous substances such as cleaning materials in a locked place using a safe locking method that prevents access by children. EVIDENCE: 1) The Licensing Inspector observed a bottle of Pine Cleaner labeled "keep out of reach of children" on top of a wooden cabinet in the K-Prep classroom. 2) The Licensing Inspector observed a bottle of bleach in an unlocked cabinet and a bottle of Pine Cleaner in the Two's classroom. 3) The Licensing Inspector observed a bottle of bleach, a Windex bottle, and disinfectant spray in an unlocked cabinet in the Three's classroom.

Plan of Correction: Per the Center: Retrain staff on the proper way to store cleaning supplies, cabinet will be locked and key out of reach of children.

Standard #: 22VAC40-191-60-C-2
Description: Based on a review of documentation on 12/27/2018, the center did not discontinue employment of two employees when the central registry findings for two of five staff were not obtained within 30 days of employment. EVIDENCE: 1) The record of Staff #4 (hired on 11/13/2018) did not contain the results of the central registry finding within 30 days of employment. Staff #4 was still employed on the date of inspection on 12/27/2018. 2) The record of Staff #5 (hired on 11/09/2018) did not contain the results of the central registry finding within 30 days of employment. Staff #5 was still employed on the date of inspection on 12/27/2018.

Plan of Correction: Per the Center: Upon hiring new staff, I will make sure all background checks are back. I will follow up on the two that have been mailed, but I have not received them as of yet.
