Comments:

An unannounced inspection was conducted on 12/3/18 in response to a complaint received by the Licensing office on 11/15/18. The omplaint was related to a two year old child being left alone and unsupervised on the facility's playground on 11/7/18, and to the center not being sufficiently staffed. Staff interviews were conducted. Staff and children's records were reviewed. The center's playground supervision and accountability policies and procedure, and tracking sheets and procedures were also reviewed. Based on the preponderance of evidence this complaint is deemed valid. Violations related to the complaint have been cited and are listed in the Violation Notice. Description:	Based on record reviews and staff interviews, staff did not follow the center's own policies and procedures. Evidence: 1. The center's "Transition Tracking Procedures" states that "Staff must conduct and record transitions with Face to Name Roll Call and Head Counts when transitioning children to a new area through a door or gate of departure." It also states that "If a trip involves passage through multiple doorways, a verbal Head Count (which is not recorded) should be performed each time". 2. On 11/7/18, at approximately 10:30a.m., Staff 1 and Staff 2 walked the Two?s-1 group from the playground to their classroom. To get to the Two's-1 classroom from the playground, a distance of approximately 83 feet, the group must exit the playground using the double doors, walk down a short hallway, make a right into the Movement Matters Area that leads to another small hallway that leads to the Two's-1 classroom on the right. 3. Staff # 6 stated that when leaving the playground, Staff # 1 and Staff # 2 counted their children but did not record the count on the tracking sheet at the time. Staff # 1 stated that when the children were lined up against the white wall in the hallway across from the playground door to return to the classroom, they did not get a chance to count the children as it was chaotic and other children and staff were gathered in that area. Staff # 1 and Staff # 2 did not record the Face to Name count or conduct a verbal Head Count when leaving the playground as required by center policy and procedure. 4. Child # 1 (2 years old) was left alone and unsupervised on the playground for approximately 3-5 minutes when their group returned to the classroom and until the child was found when another group arrived on the playground.
Plan of Correction:	After being informed of the incident, the Assistant Director immediately removed Teachers A and B from the classroom and placed both on administrative leave so the Center could conduct a full review. At this time, Center leadership is conducting regular observations of these procedures each day with oversight from the Regional Manager during her Center visits. As of November 15, 2018, these checks are documented with management?s oversight and time of check on the back of the Transition Tracking Sheet. The regular observations include doing spot checks of classrooms transitioning from one area to another at least weekly. The Center Director is individually speaking with each individual staff member regarding the importance of supervision of children and reviewing the error that occurred resulting in this violation and these meetings will be completed by January 4, 2019. Finally, we can confirm that both teachers responsible for the supervision lapse will not be returning to the Center.

Description:	Based on staff interviews, staff documentations and record reviews , a child under the age of 10 years was not within actual sight and sound supervision of staff. Evidence: 1. On 11/7/18 at approximately 10:30am, Child # 1 (2 years old) from the Two's -1 classroom was left on the playground, out of sight and supervision for approximately 3-5 minutes, when the group left the playground with Staff 1 and Staff 2 to return to their classroom. 2. Staff # 3 stated that while Staff 3 and Staff 4 were taking their group to the playground, they passed the Two?s-1 group in the hallway. Staff # 3 stated that when they arrived to the playground with their group, and while completing her name to face count, she walked over to the L-shaped part of the playground, where she saw Child # 1 standing alone on the platform. 3. Staff # 1 stated that as her group was walking back to the classroom, she realized by the "Movement Matters Area", an open room approximately halfway down from the playground door, that Child # 1 was not in the line. Staff # 1 stated that at that point she could not stop her group from walking towards the class and decided that she would go back to get Child # 1 after they arrived to the classroom. 4. Staff # 5 came to the playground as Staff # 3 was walking Child # 1 towards the playground door. Staff # 3 told Staff # 5 that she did not know who the child was. Staff # 5 recognized Child # 1 and took the child to the Two's-1 classroom. 5. Staff # 5 stated that upon arriving to the Two's-1 classroom, Staff # 1 and Staff # 2 "did not seem to indicate awareness that Child # 1 had been missing or left outside". Staff # 5 stated that Staff # 1 and Staff # 2 seemed "surprised and confused". 6. In a written statement Staff # 1 stated that she took complete responsibility for Child # 1 being left alone outside on the playground. 7. Staff #5 estimated that Child # 1 was outside alone on the playground for approximately 3-5 minutes.
