Comments:
An unannounced inspection was completed on 01/04/2019 from 10:00 A.M.-12:30 P.M, and 1/07/2019 from 9:00 A.M.-12:00 P.M. At the time of the inspection 82 children were in care with fifteen staff. Nine children records, eight staff records, four over the counter diaper ointments, six injury reports, injury prevention plan, emergency plan, first aid kits, emergency supplies, playground, and posted required information were reviewed. Children were observed during infant feeding, diapering, inside gross motor playtime, free play, fingerplays, story time, handwashing, lunchtime, and naptime. Standard #: 22VAC40-185-70-A
Description: Based on review of staff records, the center failed to have a complete file for all staff. Evidence: 1. Staff #4's record did not contain documentation of references being completed before employment. 2. Staff # 4 date of hire was 12/03/2018. 3. Staff verified that reference checks were not in the staff records.

Plan of Correction: Administration will audit all records to make sure all required documentation is complete and that staff references are on file before staff begin employment.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center failed to ensure that all hazardous materials were kept in a locked location. Evidence: 1. In Classroom #4 there was a can of Food Lion Brand Country Apple Disinfectant Spray on the top shelf of a toy storage cabinet. 2. The warning label stated "keep out of reach of children", and "hazardous to animals and humans." 3. Staff verified that the Disinfectant Spray was not in a locked location.

Plan of Correction: Staff immediately placed the disinfectant spray into a locked location, and director will review the policy regarding hazardous materials.

Standard #: 22VAC40-185-520-C
Description: Based on review of diaper ointments, the center failed to follow all requirements for applying diaper ointment. Evidence: 1. In the Infant Toddler Building staff document when they applied diaper ointment on the parent information sheet that goes home daily. The documentation did not include the frequency of application and any adverse reactions observed. 2. Staff verified that they did keep diaper documentation on site, and that the documentation did not include the frequency of application and any adverse reactions observed.

Plan of Correction: The program director will review with all staff the requirements for applying diaper ointment, and make sure all classrooms applying diaper cream have documentation forms available.
