The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing department on November 11, 2018 regarding allegations of diapering and injury records. The inspection was completed on January 3, 2019. Interviews with staff were completed, observations were made in the center, and documentation was reviewed. The preponderance of the evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, there was a violation found during the inspection unrelated to the allegations. See the violation notice for more details. Standard #: 22VAC40-185-350-C Complaint related: No Description: Based on information obtained during the inspection, the center did not ensure staff to children ratio requirements applicable to the youngest child in the group were followed when children were regularly in mixed age groups. Evidence: 1. The licensing inspector observed a staff to child ratio of 9:2 in the infant and toddler room. The licensing inspector conducted staff interviews and reviewed child records to determine that one of the children in the group was under 16 months old. Child #2's date of birth is 11/19/17. The ratio should have been 4:1. 2. Staff interviewed reported that on 11/26/18, the ratio in the infant and toddler group was approximately 1:11 for approximately four hours. Plan of Correction: Administration will ensure that ratios are met in each group of children at all times.
