An unannounced monitoring inspection was conducted from 9:30 am - 12:30 pm with the center director. There were 32 children, ranging from two months to five years of age, present with six staff supervising. Children were observed listening to stories, watching an age appropriate video, and eating snack. Snack served today was whole wheat tortillas and orange juice. Four children's records were reviewed and in compliance. Three staff records were reviewed. The emergency escape drill log was observed and in compliance. There were no medications to review. Standard #: 22VAC40-185-160-C Description: Based on a review of four staff files, and an interview with the director, it was determined that repeat tuberculosis (TB) test/screenings were not completed every two years. Evidence: The TB test for staff A is dated: 10/16/16. Plan of Correction: Will obtain a new TB test no later than 1/18/2019.
Standard #: 22VAC40-191-60-C-2 Description: Based on review of four staff records, and an interview with the director, it was determined that not all staff had Central Registry Search results within thirty days of employment. Evidence: Staff A (Date of hire: 4/18/2011, did not have documentation of a Central Registry search report on file. Plan of Correction: Will obtain an updated Central Registry search.
