63.2 Facilities & Programs.
An unannounced renewal inspection was completed on 01/10/2019 from 11:00 A.M.-4:00 P.M. At the time of the inspection 58 children were in care with eight staff and two administrators present. Seven children records, six staff records, two board member records, five medications and required documentation, seven injury reports, evacuation drills, first aid kits, emergency supplies, playground, injury prevention plan, playground safety plan, and posted required information were reviewed. Children were observed during storytime, handwashing, lunchtime, half day dismissal, outside playtime, naptime, snack, and after care activities. Standard #: 22VAC40-185-280-B Description: Based on observation, the center failed to ensure that all hazardous materials were kept in a locked location. Evidence: 1. In the Main Building in the Primary #2 Classroom there was a container of Radiance Disinfectant Wipes on a shelf in the restroom. 2. The warning label stated "keep out of the reach of children", and "hazardous to humans and domestic animals". 3. Staff verified that the disinfectant wipes were not in a locked location. Plan of Correction: Staff immediately moved the disinfectants to a locked location. The licensing coordinator will review policies pertaining to hazardous materials.
Standard #: 22VAC40-185-550-M Description: Based on review of injury reports, the center failed to ensure that all required information was on completed on the reports. Evidence: Injury report dated 12/13/18 did not include the time and date the parent was notified of the accident or injury. Plan of Correction: The Licensing Coordinator will review the requirements for completing injury/accident reports will all staff.
