Comments:
An unannounced monitoring inspection was conducted with the director beginning at 7:10 AM and ending at 8:53 AM. There were 13 children, ages 5 through 9 years old, in the care of three staff members. The children were observed playing with building blocks and dolls, drawing and coloring, and eating breakfast. Hand washing procedures were observed. Ten children's records were reviewed. Three staff records were reviewed and in compliance. Seven medications were reviewed and in compliance. The first aid kit, flashlight, and weather-band radio were observed and in compliance. The emergency escape drill log was reviewed and in compliance. The most recent fire inspection was dated 9/5/18. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of 10 children's records, it was determined that not all of the children's records were complete. Child A was missing the name, address, and phone number for a secondary emergency contact. Child B was missing an address for a secondary emergency contact.

Plan of Correction: The director stated that moving forward the staff will review the children's records more carefully to ensure that they are complete. The missing information will be obtained from the parents as soon as possible.
