An unannounced monitoring inspection was conducted on 01/08/2019 from 11:45 am - 2:15pm. At the time of entrance there were 38 children in care with six staff. The children ranged in age from infant - five years. Children were observed eating lunch and napping. Records reviewed for three staff and eight children in care. The violations are listed on the violation notice and were reviewed with the program director at the conclusion of the inspection.

Standard #: 22VAC40-185-60-A Description: Based on record review, the center failed to ensure that children's records contain all of the required elements. Evidence: The record for child 4 lacked a work phone number for one parent's place of employment. Plan of Correction: Parent employment information was obtained upon pick up.
Standard #: 22VAC40-185-280-B Description: Based on observation, the center failed to ensure that hazardous substances shall be kept in a locked place using a safe locking method that prevents access by children. Evidence: A bottle of hand sanitizer was observed on a table in the four-year old classroom. The label on this product indicated the contents as a hazardous chemical. Plan of Correction: Sanitizer was locked up and teacher was retrained on hazardous chemicals.
Standard #: 22VAC40-185-350-F Description: Based on record review, the center failed to obtain the parent's written permission prior to assignment of a child to a different age group with a higher staff-to-child ratio, if such placement is more appropriate for the child's developmental level. Evidence: 1. Child 3, who is under the age of two years, was observed in the two-year old classroom with a staff-to-child ratio of 1:8. At the time of observation, there were 11 children in care with two staff. a. The staff-to-child ratio for children under the age of two is 1:5 2. The director stated that child 3 had been moved into the two-year old classroom as this placement was more appropriate based on the developmental level of child 3. However, the written assessment and parental authorization for placement had not been signed by the parent of child 3. Plan of Correction: Parent permission was obtained on pickup.
Standard #: 22VAC40-185-500-A Description: Based on observation the center failed to ensure that children shall wash their hands with soap and warm running water after toileting and that staff shall wash their hands with soap and running water before and after helping a child use the toilet or a diaper change and after any contact with body fluids. Evidence: 1. The inspector observed staff 1 change the pull-up of child 1 which included a thorough cleaning of the genital area of child 1. a. Staff 1 did not wash her hands after this diaper change. 2. Staff 1 called child 2 into the bathroom for a diaper change. Before changing child 2's pull-up, staff 1 cleaned child 2's running nose. Afterward, staff 1 changed the pull-up of child 2. a. Staff 1 did not wash her hands after cleaning child 2's nose or changing the pull-up of child 2. b. Child 2 did not wash her hands after the change of her pull-up. 3. Staff 1 then put a pull-up on child 3 who had just finished using the toilet. a. Staff 1 did not wash her hands after assisting child 3 with toileting in the bathroom. Staff 1 left the bathroom and went into the classroom and began assisting the children in the two-year old classroom. b. Child 3 did not wash her hands after toileting. Plan of Correction: Staff member was retrained on proper hand washing. All staff members were retrained on proper hand washing.
Standard #: 22VAC40-185-500-B Description: Based on observation, the center failed to ensure that for children younger than three years, diapering shall be completed on a changing table or countertop designated for changing. Evidence: 1. The inspector observed staff 1 change the pull-ups of two children in the bathroom while the children were standing up. a. Both of the children were two-years of age. Plan of Correction: Staff who change diapers were retrained on proper diapering procedures.
Standard #: 22VAC40-185-560-K Description: Based on observation and interview, the center failed to ensure that tables shall be sanitized after each use for feeding. Evidence: 1. The inspector observed seven two-year old children playing with manipulative toys on the two tables in the classroom. Both tables had crumbs and what appeared to be dried liquid spills on them. 2. Staff 1 confirmed that the tables had not been sanitized after completion of lunch. Plan of Correction: The teachers were reminded about health and safety issues concerning children and re-taught how to clean and sanitize the tables.
Standard #: 22VAC40-185-560-N Description: Based on observation, the center failed to ensure that no child shall be allowed to drink or eat while walking around. Evidence: 1. Child 5 was observed walking around the two-year old class while drinking from a sippy cup of juice. a. This class was observed at 11:50am. Plan of Correction: The children will continue to be redirected to table and teachers trained on safety issues in classroom.
