A Subsidy Health and Safety inspection was conducted on 01/17/2019 to review supplemental health and safety requirements. Present during the inspection were 19 children, ages 5 months-4 years, in the care of 7 staff. The inspector reviewed 9 children's records and 7 staff records during the inspection. The staff stated that there was no medication being administrated at this time. The children were observed in the following activities: free choice activities, lunch and nap. Proper diapering, hand washing and table sanitization procedures were also observed. The inspector reviewed the emergency preparedness plans and the emergency supplies during the inspection. The licensing inspector discussed the license renewal process with the staff. A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice. Standard #: 22VAC40-665-650-E Description: Based on document review and discussion with the staff, the facility failed to develop and implement a written policy and procedure that describes how the vendor will ensure that each group of children receives care by consistent staff or team of staff members. Evidence: The facility administrator stated there was not a written policy and procedures that describe how vendor will ensure that each group of children receives care by consistent staff or team of staff members. Plan of Correction: A written policy and procedures for consistent care will be developed.
Standard #: 22VAC40-665-770-B-5 Description: Based on document review and discussion with the staff, the facility failed to develop procedures for continuity of operations. Evidence: The program administrator stated that there were no procedures for continuity of operations. Plan of Correction: Procedures for continuity of operations will be developed.
Standard #: 22VAC40-665-780-A-3 Description: Based on document review and discussion with staff, the facility failed to ensure that lockdown procedures shall be practiced at least annually. Evidence: There was no documentation that lockdown procedures were practiced in 2018. Plan of Correction: A lockdown drill will be done and the model drill log form will be used to document the lockdown drill.
