63.2(17) License & Registration Procedures

An unannounced focused monitoring inspection was conducted to review staff records for 7 sites operated by Hampton Roads Community Action Program. The records sample size consisted of 7 staff records for the program operated at Churchland Head Start. Areas of non-compliance are identified on the violation notice. Standard #: 22VAC40-185-160-C
Description: Based on a review of records and interview, the licensee did not ensure that each staff obtains the results of a follow-up Tuberculosis (TB) screening every 2 years from the date of their original TB screening upon hire. Evidence: 1. Seven staff records were reviewed. 2. The record for staff #1 contained the most recent results of a Tuberculosis screening which were dated 11/16/16 - more than 2 years ago.

Plan of Correction: Staff #7 went to have the screening conducted during the inspection. This was an oversight. Staff will obtain follow up TB screenings at hire and at least every 2 years thereafter.

Description: Based on a review of 7 staff records and interview, the licensee could not demonstrate that each employee of the program submitted to fingerprinting prior to October 30, 2018. Evidence: 1. Seven staff records were reviewed. 2. The record for Staff #1 did not contain results of the required Finger print based Criminal Record Review. Rather, another national criminal record review dated 12/28/16, not through the designated vendor (Fieldprint), had been obtained in error. 3. Fingerprint / National Criminal Record Checks were required to be conducted through Fieldprint prior to October 30, 2018.

Plan of Correction: The required Fingerprint / National Criminal Record Check will be requested next week and results will be filed in the record upon receipt. Future Fingerprint / National Criminal Record Checks for new employees will be conducted through Fieldprint prior to hiring them.
