Comments:
The unannounced non-mandated inspection was completed on 01/09/2019 from 2:30 P.M.- 3:35 P.M. The inspection was in response to a complaint that was received in the Valley Licensing Office on 01/08/2018 in regards to the terms of the license. Interviews with staff, attendance records, staff time sheets, one child record, was reviewed. The information gathered during the investigation does support the allegation, so the allegation is determined to be " valid". The exceptions to compliance are noted on the violation notice. Complaint related: Yes
Description: Based on review of children records, attendance records, and interviews, the center failed to operate within the terms of their current license issued by the department. Evidence: 1. The center is currently licensed for children ages 2 years of age thru 5 years 11 months of age. Child A was enrolled in the center on 11/28/2018 with first documented day of attendance being 12/03/2018, and was 22 months of age. Date of birth 02/15/2017. 2. Staff verified that Child A had been enrolled in the center on 11/28/2018 and was in attendance from 12/03/2018 thru 01/08/2019. Staff also verified the age of Child A to be 22 months and his date of birth was 02/15/2017.

Plan of Correction: The child was un-enrolled effective 01/08/2019 and will be returning when he turns 2. No children younger than two will be enrolled.

Complaint related: Yes
Description: Based on review of attendance sheets, time sheets, and interview with staff, the center failed to maintain ratio applicable to the youngest child in the group. Evidence: 1. During 12/3/2018 thru 01/08/19, in the group of children ranging in age from 22 months to 5 years of age with the youngest child's date of birth 02/15/2017, the center did not follow the required ratio applicable to the youngest child in the group. The required ratio for the group was 1:4. The following days were determined that the center was out of ratio with only four staff on site between the hours of 2:00 P.M.-6:00 P.M. 2. On 12/03/2018 twenty-seven (27) children were in attendance. 3. On 12/05/2018 twenty-four (24) children were in attendance. 4. On 12/12/2018 twenty-four (24) children were in attendance. 5. On 12/13/2018 twenty-five (25) children were in attendance. 6. On 12/14/2018 twenty-three (23) children were in attendance. 7. On 12/17/2018 twenty-two (22) children were in attendance. 8. On 12/18/2018 twenty-one (21) children were in attendance. 9. On 12/19/2018 twenty-two (22) children were in attendance. 10. On 01/02/2019 twenty-five (25) children were in attendance. 11. On 01/03/2019 twenty-six (26) children were in attendance. 12. On 01/04/2019 twenty-two (22) children were in attendance. 13. On 01/07/2019 twenty-one (21) children were in attendance. 14. On 01/08/2019 twenty-five (25) children were in attendance. 15. Staff verified that the youngest child in the group was 22 months old and that there were only four staff members working on site in the afternoons between the hours of 2:00 P.M.-6:00 P.M.

Plan of Correction: The parents had signed off on a two year old ratio, but was not applicable in this case. There are three teachers present in the afternoon, so a 1:8 ratio is able to be followed. In the case where there are more than 24 children another staff member will stay with stay until they are within ratio. We will be enrolling any child under the age of 2 in the future.
