Technical Assistance:
Fee sent in with the renewal application was applied to the 2018 installment fee and an additional fee will need to be submitted to complete renewal application. Background checks for staff were reviewed at the Parks and Recreation central location on a different day.

Comments:
An unannounced renewal study was conducted today. Ten children were present with two staff and were within rations for supervision. Children were singing and hand washing prior to snack, eating with teachers and free play. Room is well equipped with age appropriate materials. No medication was reported to be on-site today. Certifications for MAT, first aid and CPR are current as well as emergency preparation for drills and supplies. Reviewed three children's records. New injury form should include how the parent was notified and the time. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-60-A
Description: Based on record review, 1 of 2 children's records was incomplete. Evidence: Child #1 was missing an address for a parent.

Plan of Correction: Center staff will have the parent complete the missing information.

Standard #: 22VAC40-185-560-G
Description: Based on observation, food from home is not labeled and dated. Evidence: A random sample of lunch boxes found three that were not properly labeled.

Plan of Correction: Center staff will remind parents to date and label lunch bags as well as check themselves.
