Comments:
An unannounced monitoring inspection was conducted on January 23, 2019 from approximately 9:45 am to 1:10 pm. Ten staff were present with sixty-three children. Additional staff were present for administrative and kitchen duties. Seven children's records and five staff records were reviewed.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on review, facility did not ensure 1 of 7 children's records has a physical examination within a month of beginning attendance. Evidence: On January 23, 2019, the record for child #4 (start date May 7, 2018) does not have a physical.

Plan of Correction: Center Director will give guardian until next Friday to provide the physical or his care will be suspended is provided.

Standard #: 22VAC40-185-60-A
Description: Based on review, facility did not ensure 1 of 7 children's records has all required information documented in a child's record. Evidence: On January 23, 2019, the record for child #4 (start date May 7, 2018) does not have physician's information (name and phone number), allergy information and documentation of viewing proof of child's identity/age.

Plan of Correction: Center Director will speak with guardian today and items will be corrected.

Standard #: 22VAC40-185-260-A
Description: Based on review, facility did not ensure there is an annual fire inspection report from the appropriate fire official having jurisdiction. Evidence: On January 23, 2019, the fire inspection report was dated September 1, 2017 which is more than a year old.

Plan of Correction: Center Director will contact the fire department and request a visit.

Standard #: 22VAC40-185-270-A
Description: Based on observation, facility did not ensure areas and equipment of the center are maintained in a clean, safe and operable condition. Evidence: On January 23, 2019, the following was observed: 1. There is peeling paint on the walls and window sill in the 3's classroom. 2. There is peeling paint along the baseboards in the infant and toddler classrooms. 3. There are two screws protruding approximately 1" from the baseboard in the toddler classroom on either side of the child's sofa. 4. The screen is torn and separated along two edges from the window in the infant classroom. The window was open approximately 1" for air.

Plan of Correction: Center Director will work with the facilities team to have entire center painted. Screws will be removed immediately. The windows will not be operated until the screens are properly in place.
