Comments:
An unannounced renewal inspection was conducted on 1/17/19 from 10:30 am until 1:30 pm to review the licensing health and safety standards. At the time of the inspection 24 children were present with four staff. The sample size consisted of five children's records and five staffs' records. Staff and children were observed during free indoor play, bathroom break, hand washing, diapering, lunch, clean-up, nap, transitions, and behavioral guidance. The violations from the previous inspections during this licensing period were reviewed for current compliance. No repeat violations were found. Violations found during this inspection are documented on the violation notice.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, the center failed to obtain all the required components for one out of five children's records. Evidence: The record for child 3 did not contain the work phone number for either parent.

Plan of Correction: The phone number was obtained the same day as the inspection.

Standard #: 22VAC40-185-240-C
Description: Based on record review, the center failed to ensure all staff completed at least 16 hours of annual training. Evidence: 1. The record for staff 1 only contained documentation for 14 hours of training. 2. The record for staff 5 only contained documentation for 13.5 hours of training.

Plan of Correction: The director will ensure that each staff member completes at least 16 hours of annual training each year in the future.

Standard #: 22VAC40-185-540-C
Description: Based on a review of the first aid kit and interview, the center failed to ensure the first aid kit contained all the required items. Evidence: 1. The first aid kit did not contain tweezers or scissors. 2. The acting director was unable to locate the tweezers or scissors that go in the first aid kit.

Plan of Correction: The items were at the center in the director's cabinet. They were put into the first aid kit. Staff will be reminded to always return items to the first aid kit so that all staff know where they are at all times.
