Standard #: 22VAC40-185-130-A Description: Based on record review and interview, the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Evidence: 1. Child 1 has an enrollment date of 9/4/18. Documentation of immunizations was not obtained by the center until 9/12/18, which is after the child's first day of attendance. 2. Child 2 has an enrollment date of 9/4/18. Documentation of immunizations was not obtained by the center until 9/20/18, which is after the child's first day of attendance. 3. The center director confirmed that immunizations for child 1 and child 2 were not obtained until after the child's first day of attendance. Plan of Correction: Going forward, immunization records will be obtained before the child is permitted to begin school.
Standard #: 22VAC40-185-510-C Description: Based on a review of medication and interview, the center did not ensure that procedures for administering medication shall include methods to prevent use of outdated medication. Evidence: 1. There was an albuterol inhaler with an expiration date of December 2018 located in the center. 2. The center director confirmed that the medication was expired and had not been returned to the parent. Plan of Correction: The expired medication will be returned the parent on Monday, 1/28/19, and new medication will be requested if needed.
