Comments:

A Subsidy Health and Safety inspection was conducted on 1/17/19 from 10:30 am until 1:30 pm to review the supplemental health and safety requirements. At the time of the inspection 24 children were present with four staff. The sample size consisted of five children's records and five staffs' records. Staff and children were observed during free indoor play, bathroom break, hand washing, diapering, lunch, clean-up, nap, transitions, and behavioral guidance. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Description:	Based on record review and interview, the center failed to ensure all new staff receive orientation training on specific topics within seven days of the date of employment. Evidence: 1. Staff 4 was observed in the classroom with children. 2. The record for staff 4 documents the start date as 10/30/18. The record contained no documentation of completing training on food allergies. 3. The director stated she had not provided training on food allergies with staff 4.
Plan of Correction:	Training on food allergies will be completed with staff 4 and all future new staff upon hire.

Description:	Based on interview, the center failed to implement a written policy and procedure that describes how the vendor will ensure that each group of children receives care by consistent staff or team of staff members. Evidence: The acting director stated they have no new written policies and procedures.
Plan of Correction:	The center's policies and procedures will be updated to include how the center will ensure that each group of children receives care by consistent staff or team of staff members.

Description:	Based on interview, the center failed to update the emergency preparedness plan to include the new required procedural component. Evidence: The acting director stated they have not updated the emergency plan to include the new requirement effective 10/17/18 regarding continuity of operations procedures to ensure that essential functions are maintained during an emergency.
