A Subsidy Health and Safety inspection was conducted on 1/25/19 from 10:45am to 12:00pm to review supplemental health and safety requirements. There were 12 students in direct care and supervision of 7 staff members. The students were observed listening to music, using the bathroom, completing an art project and washing hands before lunch. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: 22VAC40-665-650-E Description: Based on staff interview, the center did not develop and implement a written policy and procedure that describes how the center will ensure that each group of children receives care by consistent staff or team of staff members. Evidence: Staff # 1 stated that a written policy and procedures plan was not implemented to address the need for group sizing at the center. Plan of Correction: We will prepare and implement a plan and discuss with staff.
