Comments:
An unannounced monitoring inspection was conducted today from 9:50am-1:00pm. There were 32 children (2-5yrs.) supervised by 5 direct care staff. The physical plant, 3 staff records, 5 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. There were no medications on site. Children were observed participating in circle time, learning about winter clothes and simple machines, playing in the sand table, in the home living area, and playing with magnets, animal puppets, and puzzles. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-60-A
Description: Based on record review, 5 of 5 children's records did not contain all of the required information. Evidence: 1. Child #1's (start date: 9/24/2018) record did not contain documentation of a physician's name and phone number, immunization, and physical form completed by a physician. 2. Child #2's (start date: 10/22/2018) record did not contain documentation of the father's work location and phone number, and address for either of the two emergency contact persons. 3. Child #3's (start date: 6/11/2018), and Child #4's (start date: 9/26/2018) records did not contain documentation of a complete address for either of the two emergency contact persons. 4. Child #5's (start date: 1/23/2019) record did not contain documentation of a second emergency contact person and an immunization record.

Plan of Correction: We will start reviewing the files and get the information that is missing.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances were not kept i a locked place using a safe locking method that prevents access by children. Evidence: In the Ants classroom (ages 4-5years), there was a container of Clorox wipes in an unlocked teacher cabinet.

Plan of Correction: Corrected during inspection.
