Technical Assistance:
Consultation provided on the need to protect the children for their classmates who exhibit persistent aggressive behaviors. Consultation provided on the need to make sure that medication authorizations filled out by the parent/physician is specifically giving this center the authorization to administer medication.

Comments:
An unannounced monitoring inspection was conducted today with the director and assistant director. Ninety four children were present at the end of the inspection to include a small group of schoolagers who had arrived after school. Most of the children were on their cots for nap time. For lunch, staff reported that the children were served chicken salad sandwiches, cucumbers, tomatoes, applesauce and milk. A sample size of 7 medications were reviewed for five children. Seven staff and children's files were inspected and in compliance. Documentation for injury/accidents were reviewed and complete and the center's new system of organizing medication made it easy to inspect. Please submit your plan of correction for the violation by 1/25/19. Arrival time: 12: 30 pm Departure time: 4: 25 pm

Description: Based on observation it was determined that the center is not maintained in a safe manner. Evidence: Part of the fence surrounding the school age playground has an openings that allowed a small torso template ( 3 1/2 x 6 1/4 inch rectangle) to fit through but not a large head template ( 9 inch circle) to fit through, which presents a head entrapment hazard.

Plan of Correction: The fence has been secured using zip ties. Discussed necessary repairs with the head of our maintenance department. Technician will evaluate the best method to repair or replace fence section that will alleviate any hazards. Repairs will be completed by February 1, 2019. Teachers have been reminded to maintain a safe distance for the protection of the children.
