63.2(17) License & Registration Procedures
An unannounced Monitoring Inspection was conducted on January 28, 2019 from approximately 3:05 pm until 5:00 pm. There were 16 children and 2 staff present with children. The record review sample consisted of 5 children's records and 2 staff records. The violations from the previous inspection were reviewed. Children were observed preparing for and eating snack, playing board games, and coloring. The results of the inspection were reviewed with the designated center director. There were no medications on site at this time. Areas of non-compliance are noted on the violation notice. Please contact the licensing inspector with any questions (757) 404-0568.
Standard #: 22VAC40-185-60-A Description: Based on record review and staff interviews, the licensee did not ensure to maintain and keep at the center a separate record for each child enrolled which shall contained all the required information. Evidence: During the inspection on January 28, 2019, the following records were determined to be incomplete; 1) The records for child #1 and child #7 did not include the complete home address for the emergency contact listed. 2) The record for child #2 did not include the required legal paperwork when a parent is listed as an unauthorized pick up for the child. 3) The record for child #4 did not include the phone number for one emergency contact listed. Plan of Correction: The center director will talk to the parent and ask them to update the records.
Standard #: 22VAC40-185-240-D-5 Description: Based on documentation review and staff interviews, the licensee did not ensure there shall always be at least one staff member on duty who has obtained within the last three years instruction in performing the daily health observation (DHO) of children. Evidence: During the inspection on January 28, 2019, there was not a staff on site from approximately 3:10 pm until approximately 4: 17 pm with DHO. Staff #1 and staff #2 were the only staff on site and they confirmed they have not obtained DHO training. At approximately 4:17 pm, staff #3 arrived at the center and they have documented DHO training. Plan of Correction: The center director will ensure the staff get the training.
Standard #: 22VAC40-185-560-F Description: Based on observation and staff interviews, the licensee did not ensure that when the center choose to provide meals or snacks, they must list any substituted food on the menu. Evidence: During the inspection on January 28, 2019, the children were served strawberries, blueberries and graham crackers. The menu posted listed pizza bagels as the designated snack for today and the menu was not updated to reflect what was served. The center staff confirmed they were not going to change the menu because they were serving what their supervisor told her to serve for snack. Plan of Correction: The Operations Manger will advise the center director and staff to make changes to the physical menu and in advance when possible.
