- Technical Assistance:
Background check procedures and records for staff who have transferred from another KinderCare center were discussed.
An unannounced monitoring inspection was conducted on 1/24/19 from 11:10am until 2:20pm. At the time of the tour, there were 43 children in care with 8 staff present. One school-aged child arrived at the center with one staff during the inspection. A sample of 6 children's records and 5 staff records were reviewed. Children were observed participating in morning activities, eating lunch and resting quietly during nap time. Hand washing, infant feeding and diapering and table sanitizing procedures were also observed. Medication, first aid and emergency supplies, the emergency preparedness plan, documentation of emergency practice drills, children's injury reports and center postings were reviewed. Two violations in the area of administration are identified on the violation notice and were discussed with the center director during the exit interview.
Standard #: 22VAC40-185-130-A Description: Based on record review and interview, it was determined that the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Evidence: 1. The record for child 1 contains documentation of immunizations from when the child was 4 months old. Child 1 was 2 years old at the time of enrollment (8/28/18) and there is no documentation that the child has received immunizations beyond 4 months of age. 2. The center director confirmed that the center did not obtain documentation of the most recent immunizations for child 1 at the time of enrollment and have not obtained updated immunizations since that time. Plan of Correction: The center responded with the following: Child's parent was informed at close of business of the need to update required paperwork. Child is unable to return until physical with updated immunizations are received.
Standard #: 22VAC40-185-140-A Description: Based on record review and interview, the center did not ensure that each child shall have a physical examination by or under the direction of a physician before the child's attendance or within one month after attendance. Evidence: 1. The record for child 1 does not contain documentation of a physical examination. Child 1 has a date of enrollment of 8/28/18. Child 1 was in care on the day of the inspection. 2. The record for child 2 does not contain documentation of a physical examination. Child 2 has a date of enrollment of 12/9/18. Child 2 was in care on the day of the inspection. 3. The center director confirmed that the records for child 1 and child 2 do not contain documentation of a physical examination. Plan of Correction: The center responded with the following: Child 1's parent was informed at close of business of the need to update required paperwork. Child is unable to return until physical with updated immunizations are received. Child 2's parent was informed during visit and contacted pediatrician for current physical paperwork to be sent via fax. Child cannot return until paperwork is received.
