Technical Assistance:
We discussed the requirements for updating immunization records. Also, we talked about the requirements for documenting parent notification of injuries, hand washing routines when diapering children, and infant records when children are fed formula.

Comments:
Thank you for your assistance during today's unannounced monitoring inspection conducted from 9:57 AM to 5:00 PM. Today, there were sixty-seven children, ages infancy through five years, in care during the day with twelve staff. It was reported that two school age children received supervision before school this morning. Fourteen children attended after school activities with one staff member. I viewed program activities, daily routines, infant care and records, staffing, supervision, interactions with children, posted information, equipment, nutrition, meals/snacks, medication, diapering procedures, six records for children, injury records, emergency supplies and drill records, eleven staff records, staff qualifications, staff training, agency inspection reports, insurance, indoor and outdoor areas. The children have enjoyed a variety of age appropriate toys and play equipment. Regular activities have included art, stories, group discussions, music, and activity time with the adult center. Staff were observed providing frequent interactions with children and responding to individual needs for care and attention. Organized record keeping systems have been utilized. Standard #: 22VAC40-185-130-C
Description: Based on a review of six records for children, and an interview with administrative staff, the center failed to obtain documentation of additional immunizations at least once between one child's fourth and sixth birthday. Evidence: Child 6 became six years old earlier in January 2019 and the last updated immunization record was obtained on August 24, 2016.

Plan of Correction: The executive director immediately contacted the parent and an updated immunization report from the health care provider was received by FAX this afternoon on January 28, 2019. Administrative staff will review the other records of school age children to make sure all required updated immunization records were obtained for each file. Administrative staff will request the updated immunization records as children prepare for kindergarten.
